There's a pattern I see constantly in the clinic: someone rehabs an ankle sprain or an irritated Achilles, does everything right, feels good, and breaks down again within a few months of returning to sport. When we dig into why, the answer is often one level down: the foot itself never got strong.
Your foot has over two dozen muscles, and a whole group of them, called the intrinsics, live entirely inside the foot. They control your arch, stabilize your toes, and fine-tune how your foot meets the ground thousands of times a day. In most adults, they're badly undertrained.
How feet get weak in the first place
You didn't neglect your feet. The modern environment did it for you:
- Shoes do the foot's job. Cushioned, supportive footwear is comfortable precisely because it takes over the work your arch and intrinsics evolved to do. Wear that support every waking hour for thirty years and those muscles stop doing much of anything, the same way an arm in a cast loses strength.
- Flat, predictable surfaces. Concrete and flooring never ask your foot to adapt. Grass, sand, trails, and uneven ground constantly challenge the foot's small stabilizers. Sidewalks never do.
- Rehab skips it. Even good ankle and Achilles rehab tends to train the big muscles, calves and glutes, and never asks the foot itself to do anything.
Why this sabotages ankle and Achilles rehab
Every step you take, force passes from the ground, through your foot, up through the ankle and Achilles, and into the rest of you. The foot is the first spring and the first filter in that chain.
When it's weak, two things happen. First, the structures above it, especially the Achilles, absorb load the foot should have shared. Second, the foot gives the brain fuzzy information about the ground, so your balance and control degrade in ways you don't consciously notice. A stiff, weak foot is a reliable way to keep re-spraining an ankle or re-irritating a tendon that you swore you rehabbed properly. If that sounds familiar, there are four other common reasons Achilles rehab stalls worth ruling out as well.
What actually strengthens a foot
The good news is that feet respond to training like anything else. Here are the broad principles, in the order that tends to work:
- Wake it up before you load it. Feet that have lived in shoes for decades benefit from sensory work first: time barefoot on varied surfaces, learning to feel and spread the toes, and restoring big-toe mobility. Boring and cheap, but it works.
- Then make it work. The foot needs real strength work, not just wiggling the toes. That means progressively loaded exercises where the arch and toes actively grip, support, and push. Most people have never done a single hard set for their feet in their lives.
- Then connect it to everything above. Foot strength only pays off when it's integrated with the calf, the ankle, and eventually springy, fast movement. A strong foot that never learns to work during walking, running, and jumping won't carry over to the activities you actually care about.
That order matters: sense, strengthen, integrate. The sequencing is what most free advice gets wrong. And if it's your Achilles that keeps flaring, where exactly it hurts changes the plan too.
Common questions
Can flat feet be fixed?
The shape of your arch is largely structural, and chasing a different-looking foot isn't a useful goal. What is trainable is what the foot can do: how well it controls load, how much force it tolerates, and how well it stiffens and springs when you ask it to. Plenty of people with flat feet are strong, fast and pain-free. Function matters considerably more than the silhouette.
How long before foot strength work makes a difference?
Balance and sensory changes often show up fairly quickly, within a few weeks. Genuine strength changes in muscle and tendon are slower, more like a few months of consistent work. That's a normal timeline for tissue adaptation rather than a sign it isn't working.
Is toe yoga and spreading my toes enough?
It's a reasonable starting point, and for a foot that's been asleep for twenty years it's genuinely useful. But it's the warm-up to the job, not the job itself. Feet respond to progressive loading like every other part of the body, and if the work never gets harder, adaptation stops.
